Alzheimer’s Diagnostic Discrepancies Highlight Health Disparities for Black Americans
Diagnostic tools calibrated on predominantly white populations may fail to accurately detect Alzheimer's biomarkers in Black patients, highlighting the need for inclusive research and diverse medical professionals.

Despite facing significantly higher rates of Alzheimer’s disease, Black and Latinx older adults are less likely to show positive results on amyloid PET scans—a primary diagnostic tool used to identify the condition. A growing body of research highlights this critical disconnect, raising serious concerns about how brain health diagnostic tools are developed, tested, and applied across diverse populations.
Amyloid PET scans detect the accumulation of beta-amyloid plaques in the brain, a hallmark indicator long associated with Alzheimer's disease. However, because these imaging standards were largely normalized using data from predominantly white study participants, they may fail to capture the distinct biological pathways through which cognitive decline manifests in Black patients.
A Mismatch in Diagnostic Standards
Epidemiological data consistently shows that Black Americans are roughly twice as likely to develop Alzheimer’s disease and other forms of dementia compared to their white peers. Yet, when evaluated using standard amyloid PET imaging, Black individuals frequently present with lower levels of detectable amyloid plaques, even while exhibiting clear clinical symptoms of cognitive impairment.
This discrepancy suggests that cognitive decline in Black communities may often be driven by alternative or complex underlying factors, such as vascular brain health issues, mixed-dementia pathologies, or socio-environmental stressors. When clinical diagnostic criteria rely heavily on a single biomarker calibrated to white populations, Black patients risk being misdiagnosed or excluded from newly emerging targeted therapies.
The Urgent Need for Workforce Representation
The persistent gaps in Alzheimer's diagnostics underscore a broader imperative within medical research: the need for diverse representation among clinicians, neuroscientists, and clinical trial investigators. Historically Black Colleges and Universities (HBCUs) play a pivotal role in training the next generation of Black medical professionals and researchers who are uniquely equipped to address these racial disparities.
By advancing research that accounts for health equity, HBCU-trained scientists and physicians are at the forefront of championing more inclusive clinical trials. Developing diagnostic tools that accurately reflect the biology of all populations is essential to ensuring that Black communities receive timely, effective, and equitable care for neurodegenerative diseases.
