Updated Cardiovascular Risk Assessment Tool Recalibrates Heart Disease Estimates for Women and Black Adults
An updated American Heart Association tool changes how cardiovascular disease is calculated for women and Black patients, shifting away from race-based metrics.

A new cardiovascular risk assessment model developed by the American Heart Association (AHA) significantly shifts how doctors evaluate heart disease risk across different demographics. Known as the PREVENT equations, the updated tool frequently estimates a higher risk of cardiovascular events for women, while reclassifying many Black adults into lower-risk categories compared to legacy risk calculators.
The findings highlight a major evolution in how clinical algorithms approach race, sex, and socioeconomic factors. Older assessment tools, such as the Pooled Cohort Equations introduced in 2013, routinely factored race directly into their calculations, often assigning higher baseline risk scores to Black patients regardless of individual health metrics.
Rethinking Race in Clinical Algorithms
The PREVENT tool marks a deliberate departure from race-based medicine by removing race as an explicit predictor variable. Instead, the algorithm incorporates kidney function indicators—such as estimated glomerular filtration rate and urine albumin-to-creatinine ratio—alongside traditional risk factors like blood pressure, cholesterol, smoking status, and diabetes. It also includes measures of social determinants of health based on zip codes.
When researchers evaluated the new model, they found that removing race reduced calculated 10-year cardiovascular risk scores for many Black individuals. While removing race from clinical algorithms aims to prevent bias, researchers note that lower predicted risk scores could also influence prescribing practices for preventive medications like statins or antihypertensives, making accurate clinical interpretation critical.
Unmasking Cardiovascular Risk in Women
While predicted risk decreased for a subset of Black adults, the PREVENT tool systematically increased cardiovascular risk estimates for women across demographic groups. Historically, standard risk models underestimated heart disease in female patients, leading to delayed diagnoses and fewer preventive interventions.
By integrating kidney metrics and broader metabolic indicators, the PREVENT calculator better captures systemic vascular health, offering a clearer picture of how heart disease develops in women. Because cardiovascular disease remains the leading cause of death among Black women, precise risk estimation is essential for delivering timely, life-saving care.
The Need for Culturally Informed Care
The shift in risk scoring underscores the vital role of Black medical professionals and HBCU-trained clinicians in navigating changes to clinical guidelines. As predictive algorithms evolve, culturally competent providers are key to ensuring that lower numerical risk scores do not lead to undertreatment or overlooked symptoms in Black communities.
Cardiovascular disease continues to disproportionately impact Black Americans, driven by systemic health inequities, structural barriers to care, and higher rates of hypertension and diabetes. Tools like the PREVENT equations provide valuable data, but equity in heart health ultimately depends on clinicians who understand both the science of risk prediction and the lived experiences of the patients they serve.
